Dentition Photos
Please upload photos of your Pet(S) teeth from the front. Both left/right, upper and lower canine teeth should be clearly visible in the photos. If uploading pictures of multiple dogs, please make sure each file includes the dog's name, so we are able to identify each individual dog's teeth.
Full Body Profile photos
Please upload a full body profile picture of your pet(s). Your pet should be standing fully upright and facing forward.
CDC Rabies Vaccination & Microchip Record (RVMR) Please upload your dog(s) CDC Rabies Vaccination and Microchip Record (RVMR) here. An RVMR is required to enter the United States - traditional Rabies Certificates, pet passports, or any other certificates are not accepted. The certificate(s) must be in English. If you have multiple dogs that require reservations for importation, each dog much have an individual RVMR uploaded here.
Rabies Titer Results Please upload your dog's Rabies vaccine titer lab work results here, if available. If you have multiple dogs that require reservations for importation, each dog much have titer results uploaded here. If no results are uploaded, we will assume you do not have Rabies vaccine titer for your dog(s).
Additional Veterinary & Vaccine Records All boarders at Pender Pet Retreat are required to be up-to-date on Rabies, Distemper, and Bordetella vaccines, negative fecal parasite testing, as well as endo- and ecto-parasite prevention. Please provide all of your pet(s) vaccination information and recent veterinary records HERE for our review. We will provide you with an estimate for any additional veterinary care required upon arrival for boarding at our facility.
